Send this first. Everything else waits on their records and their written schedule. The Whole Child Center, (201) 634-1600, Monday to Friday 9 to 5. It deliberately does not tell them how to sequence the doses. That is their call, and the questions for it are underneath.

Subject: Catch-up schedule and school medical forms for our twins
Hello, Our twin girls start preschool on September 2, and we are catching them fully up on their immunizations under Dr. Rosen's guidance. We would like to do this one dose per child per visit, roughly one dose a week. We are not in a hurry to finish, and we would rather go carefully. Four things we need: 1) Their complete immunization records, both girls. 2) An appointment before August 14. Our current one is September 10, which falls after school starts. 3) The New York OCFS-LDSS-4433 medical form completed for each of them. 4) A written, dated schedule of the remaining appointments. The school needs that written schedule on file, so it matters as much as the doses themselves. Thank you, Jake Lee 201-321-5446

Questions for the visit, not for the email

Which series are at zero doses?
Those need a first dose before September 2, so they go to the front of the queue. Everything else can be booked forward. This is the single question the September 2 date rests on.
How far apart will MMR and varicella be?
Two live vaccines given on separate days need 28 days between them. With one dose per visit they cannot land in consecutive weeks, or the second does not count and has to be repeated.
DTaP fourth dose: four months, or six?
The state's own footnote says a dose given at least four months after the third does not need repeating. Entirely their call, but worth asking, since it would move completion from February to December.
Can a nurse practitioner sign the form?
A nurse can give the dose but cannot sign the OCFS statement. A physician, physician assistant, or nurse practitioner can, which means the form does not have to wait for September 10.

Note what this one does not do. It does not ask her to accept anything. It reports a status and says what documentation is coming. A request invites a no. A record does not.

Subject: Immunization status before September 2
Hi Holly, Sharing an update on the girls' immunizations ahead of the first day. They are behind, and we are catching them fully up under their pediatrician's supervision, one dose per child per visit. Their doctor is Dr. Lawrence Rosen at The Whole Child Center in Oradell. Because DTaP requires six months between the third and fourth dose, that series completes in early 2027. Every other required series completes this autumn. They will therefore be attending as in process under 10 NYCRR 66-1.3(b). Before September 2 you will have both completed OCFS-LDSS-4433 forms and Dr. Rosen's written schedule of the remaining appointments. Let me know if you would like that packet in a particular format. To Holly, the first day groups

Time sensitive. Assignments go out the week of August 3, and the two toddler hours do not overlap, so one caregiver cannot cover both girls.

Hi Holly, There is one decision for you on the first day groups. Both of ours are in the toddler class, and the two visiting hours do not overlap, so one of us cannot cover both children. Two arrangements work on our end: 1) Both girls in the same group, with both of us there. 2) One girl in each group, with one of us staying for the two hours back to back. We would prefer the first if the class balance allows it. They lean on each other in new places, and the first morning is not where we would separate them. Whichever is easier for the classroom works for us. Thank you, Jake

Hi Holly, Closing the loop on the enrollment paperwork. Attached for both girls: 1) Medical forms, completed by their pediatrician. 2) Intake forms. 3) Emergency contacts, two per child. 4) The signed handbook acknowledgment page. 5) Dr. Rosen's written immunization schedule through early 2027.
